By submitting this form, I confirm that the information I have provided is accurate and that I agree to the following in connection with GT Launchpad events hosted by Green Therapeutics (“GT”):

Assumption of risk & release. I attend Events voluntarily and assume all risks of attending, including any exposure to or sampling of cannabis products. To the fullest extent permitted by Nevada law, I release and hold harmless GT, its affiliates, owners, employees, and the Event venue from any claims, damages, or injuries arising from my attendance, except for harm caused by willful misconduct.

Photo, video & likeness release. I grant GT permission to capture and use photographs, video, audio, and my name and likeness from the Event for GT’s marketing and promotional purposes, in any media now or in the future, without compensation or further approval.
